  1. It’s reasonably common. They are usually bolts but sometimes you get a threaded axles with nuts. Generally on cheaper bottom brackets. The same crank extractor should work, but you get the odd design that doesn’t fit nicely. Just make sure it screws fully into the crank as the alloy can strip.

  2. Lower end BBs have boy BBs. You take off the crank arm like any other square taper. With a crank puller.
