I’ve never seen a bolt that was two separate pieces in a square taper crank set before or in any crank set for that matter. The bolt head did not break off. Does anyone have a good idea on how to remove this?
It’s reasonably common. They are usually bolts but sometimes you get a threaded axles with nuts. Generally on cheaper bottom brackets. The same crank extractor should work, but you get the odd design that doesn’t fit nicely. Just make sure it screws fully into the crank as the alloy can strip.

Lower end BBs have boy BBs. You take off the crank arm like any other square taper. With a crank puller.
