    Pedals are side-specific – the left pedal has a reverse thread. It’s surprisingly common to see people force the right pedal on the left side and vise versa. This significantly weakens the thread, and switching back to the correct sides after will probably result in failure like this.

  2. How are the bearings in the pedals?

    They could’ve gotten stuck somehow and as you pedaled it kept tightening them to the crank until it the threads stripped. I’m assuming this is based on your description of the cranks being old.

    Usually when I see this thing it’s over tightening mixed with misaligning the threads when you’re installing.
