  1. When you say crank you are referring to the front gear (the chainring)? Need more information to address this issue.

    The most likely issue why it can’t shift into certain gear is your limits and cable tension need to be adjusted. Look for videos how to tune a rear derailleur.

  2. It’s a combination of your limit screws and the cable tension.

    The limit to stop it falling off the top is the easy one – don’t use the shifter but pull the cable under the downtube (if it’s not internal routing) tight with your hand – and turn the low limit screw until you can’t get on to the biggest cog – then back it off a little so you can but the chain won’t go over the top.

    The other limit is similar but this time change into the highest gear with the shifter, check the cable is fairly slack and adjust the other limit screw so you can get on to the smallest cog but not any further.

    Then it’s just a case of checking the cable tension – click the lever once and it should go up to the next cog, and click the other way it should go down – turn the barrel adjuster to increase the tension if it won’t go up or decrease if it won’t go down.

    As long as the hanger is straight that will do it and just takes a minute with practice.

  3. Looks like it’s in a bike shop. Get a mechanic to look at it.

    Frankly, it looks like the hanger is bent in a hair (look how close it is on the bottom of that freewheel).

    I would put a hanger alignment tool on it before doing anything else.
