
Pad lined up fine with the rim when it was in the middle tension hole, but there wasn’t enough tension so I raised it to the top of the three holes.
Tension is good but now I can’t get the pad to hit the rim…. Do some pad / brake / hole combinations just not work?

Put it back how it was. Then use the tiny grub screw (aka set screw) on the right side of your picture to adjust tension. Screw it in clockwise to add tension.
What is this ‘tension hole’? tension is set with cable length.
Maybe a pick of the entire brake would be helpful to diagnose.
Does your rim diameter actually match the fork?
Those three holes are just for pad height alignment, not tension. Use the little spring tension screw on the arm to dial in spring strength, then set cable tension with the barrel adjuster.
Did you change from cantilever to V brakes?
It’s the linkage. My XTR ones did the same thing. It is NOT a spring adjustment issue. I fixed mine my removing it, completely disassembling it, cleaning and luring the linkage. When I reinstalled it, I did not tighten it on the canti boss until I had the pad flat on the rim….then I tightened it. Works perfect.
