The cassette is either 1) not full on the freehub body 2) there is a spacer behind the cassette that needs to be removed 3) the lock ring threads are stripped.
I would see if there is a spacer behind the cassette and if not inspect the freehub threads and lock ring threads. If it looks like most of the threads on the lock ring are okay but one or two are stripped, then it’s the cassette not sitting on the freehub enough. If many of the threads are stripped, then it’s a lock ring/freehub problem (likely only the freehub if it’s an aluminum freehub body).
Also remember, 40nm is not crazy tight. You do not need to put the force of God into tightening a cassette lock ring.

I’d say you have pulled the thread on the locking nut….. Ask me how I know 🤦
If you’re lucky the locking nut is ali and the free hub is hardened steel so the free hub is hopefully not damaged. You’ll need to either source a new locking nut or buy a whole new cassette to get the locking nut.
