I’ve regreased and reinstalled BB, crank, chainrings, and pedals with no change. I’ve also tried other pedals, which is the only thing I can easily swap out (no other compatible wheels). I haven’t reproduced the noise without actually riding.
I mostly notice this noise when I’m in the small ring, but it does happen in the big ring as well.
Seems to be tied to crank revolutions but not 1:1.
Sometimes, I get a much more pronounced creak (longer creaking noise, less clicky), which turns back into this normal click after brief backpedaling.
It’s a thread-together BB86 bottom bracket, am I wrong that replacing the BB is a reasonable next step at this point? The bearings don’t feel bad in my hands.

Can you throw in a different rear wheel and listen? If it remains, swap the bb

Grease the seatpost. Bike frames echo alot, otherwise try replacing the bb

In my case, one of the derailleur hanger had a loose screw.
Battled with that noise for months, eventually had to bring the bike to my LBS.
They found it quite fast. Fixed everything !

It’s a long shot, but if you have a SRAM chain, make sure the master link is not upside down.

Believe it or not I had a two creaks I could not locate. I started systematically greasing.
1st went on the rear derailleur hanger
2nd – was the water bottle screws. Enough flex in the carbon to create a click on the pedal strokes under load.
Crazy – but did find them.

Crazy, i have the exact same sound/pattern, though mostly in the big chainring.
The bike is basically brand new, and it started after mounting new carbon wheels, with new centerlock rotors and mounting the old cassette.
I have no idea what it could be so far.

I also had a one-sided clicking noise and it almost drove me crazy.
The solution was to completely remove the seatpost, clean everything thoroughly, put it back together, and then apply a thick layer of grease to protect it from dirt.

I can’t tell what crank that is. Any chance that’s one of the Ultegra that was recalled? I remember they sent out an audio file with a clicking as a diagnostic.
