Rust under the paint. Could be structural. You gotta remove all that paint and see how bad it is. If it is structural then you need the frame repaired. A decent frame builder can sleeve in a new section or replace the seat tube entirely. How much is that tandem worth to you?

Water probably got in the seat tube and rusted it from the inside out over a long period of time.
Finally a bike meant to be photographed from the left side, and it’s photographed from the right!
Seriously though, that’s a lot of rust under that paint.
