A local machine shop could do it. You’d need to think about metal type and if you want it hardened and if you need the teeth beveled, and probably give them a drawing or your crank to measure.
You probably don’t specifically care if it’s done on a mill with a super spacer or a water jet or plasma, as long as it’s clean.
