I rotafix all my cogs and have never had this happen in 2 decades of riding.

This happened to a friend’s hub and we caught it in time before all the threads stripped.
Used some jewelers epoxy like grease on the cog and lockring threads because we couldn’t get any replacements during COVID. The epoxy cog wheel is still alive and kicking today.
