Any idea what might have caused the o-ring on the valve to mis-shape like this?
Valve has been installed for four years or so and never lost air. Now it’s losing approximately 6 PSI an hour. After investigating I found the O-ring on the valve to be misshapen. Any idea what’s might have caused it?
As for a fix, new valve? Or can I Remove the lock ring, slide a new o-ring over the valve and retighten the lock ring?

The o-ring it’s not what seals the valve, but the conical rubber the other side. The o-ring kind of keeps the lock washer in place. Probably that rubberon the inside has been deteriorating and you’ve been tightening down the valve until the point where it pushed the o-ring through. So yes, you likely need a new valve.
Just replace the valve . The back is rubber which will eventually degrade over time
If you’re losing air like that I’d clean it and retape it while I was at it
Cause: the valve is 4 years old, it’s had a good run man. Is the tape also 4 years old?
Before you buy a new valve, check whether the valve people sell a service kit or the rubber thing on its own.
The stem itself isn’t broken after all.
