It also feels like it’s kinda loose so I thought it was from the handlebars being not tight enough but I tried several times to tighten the stem cap and sanding the clamping etc. now I think it’s a problem with the compression cap but it’s very tight on. Thoughts?

Sounds like your headset bearings are fucked. Could be the lower also. Take it all apart and check them i think you’ll find they’re damaged if not completely smashed out of the race. Repair/replace then pack them back in with grease and don’t over tighten the compression cap. Then tighten the stem clamp bolts. Nothing in the bar clamp can cause this. don’t overtighten things. Also reroute your brake cable outside the bars.
