It’s not? The crank arm looks like it’s slipping off. Pull it off, clean the spindle, and reinstall it with the proper torque.

I am guessing the pinch bolts weren’t tightened enough and the crank arm started working its way off the spindle, although the preload bolt should have prevented this, unless it’s missing or stripped out. You should be able to push the crank arm back on, use the preload bolt to snug it up, then torque the two pinch bolts to spec.
