That’s not wear, that happened because of a crash.
And yes I would worry about it slighty, since you don’t know what happened and what the extent of the damage is.

Most importantly, you’re not gonna know for sure unless you take it to a carbon repair shop. A repair and repaint will run you somewhere in the neighborhood of $300-600 for something like this, based on my experience

Looks like heel rubbing from their shoe. It has gone through all the paint and has got down to the carbon fiber layer. Not great but unlikely to be a structural problem. Still should be a lot cheaper than other similar bikes.
