  1. Looks fine. I’d check the limit screws on the derailleur. Look for the park tool you tube video on how to adjust them.

  2. Speaking from relatively limited experience, but…Prob need a new chain. If that doesnt do the trick then a new cassette. That sprocket gets a lot of torque with the least amount of teeth so when your chain is stretches its most likely to jump there.

  3. Hard to tell how worn the teeth are by looking at a photo, but you can see the OP is using the smaller cogs way more than the larger ones. Smaller cogs wear out faster than the big ones…

I’m thinking it’s worn out, time for new chain and cassette.

That said, measure your chain for stretch and make sure everything is properly adjusted as it sits now.

If the chain is crazy worn, or still skips when everything is well adjusted, replace the chain and cassette.

Another thing you can try is replacing the chain alone. If it skips, then you definitely need a new cassette as well.

A larger chainring, assuming this is a 1x setup, will force you to use some of those middle cogs and give you a higher overall top gear.

  4. Smallest cog looks very worn, others don’t look that bad. Chain is probably worn as others have noted. Start with chain and try to lay off the 11T to see how it behaves. Some cheaper cassettes come with the smallest two cogs loose and the rest bolted together, you can occasionally find stray 11T cogs because of this.

  5. I would go for a new chain and cassette. Your small sprockets look way more worn than big ones. The smallest sprockets can be replaced separately. This might be a good idea if ride big in the front and small in the rear most of the time.

Skipping is the symptom of a mismatch between the chain and the gear. It might not have skipped (yet) with the prior chain, but the current chain is incompatible. Replace and start the cycle over again.

I had this happen, chain kept falling off. They removed a link and adjusted the RD, kept falling off. They replaced the chain and adjusted the DH, chain still kept falling off. I did not replace the cassette but I ended up getting a new bike lol

looks worn compared to the rest of the cassette. I’d buy a new 11T cog. That is the reason for the 11T being separate from the rest of the cassette, they know it is probably going to be the most used cog so they made it so you can just replace the cog instead of the entire cassette. There are tons of listing for 11T cogs on Amazon, it doesn’t necessarily have to be Shimano.
