    Looks like it was either crossthreaded or more likely just not torqued enough.

    The crank is done. Unless it’s expensive, just replace it.

    Clean threads. Might be cross threaded also. Hard to say with all that gunk in there. Remove and clean. Carefully thread from other side (from the inside). This will chase and repair threads.

    FYI my pedal install technique is to clean the threads of both the spindle and the crankarm as best as I can, usually with a thin rag and my fingernail.

    Then I apply a light coating of grease and screw it in by hand. If I did it correctly, it should screw in at least half way (ps left and right pedals have opposite threading).

    When I screw in the pedal, preferably the entire way, then I tightening it will a metric (not English) allen key. I think this is 6mm but it could be 8mm. You are probably not going to overtighten it so just push down as hard you can.
