No idea what is causing this – the chain doesnt sit in between the teeth initially, then it slowly gets into sync the more it rotates around. The chain is not stretched at all. Worn cassette?

Try settings up your limit screws. I’ve had the same problem because my upper limit was preventing the derailleur from being aligned with the cassette on the biggest cog
If your cable tension is correct and all other gears are shifting smoothly check your limit screw and I’d personally also adjust the b-tension screw. Your jockey could be a little bit closer to the cassette.
Mismatched wear on cassette and chain. If the chain is new, replace the cassette.
Incorrect limit screw (L) setting, and the cable tension barrel probably needs adjustment.
