The front wheel steered out while you held the bars straight. IOW the stem bolts aren’t torqued properly.

Tucked the front wheel. You weren’t following the arc of the corner and we’re going straight towards the edge of the trail and then you tapped the front brake. I’m assuming a lot here but it’s hard without sound or anything else to go on.

  5. The little rocks that get sorted out of the dirt roll down hill and end up as loose gravel. Don’t put your tires on the gravel. Stay high in the berm

Stem bolts weren’t tight enough, forks/tire turned while the bars stayed straight. The riding had nothing to do with how you fell, unfortunately just failure of the bolts. Just tighten them real good before the next ride

  7. Well you see, the goal is to stay on the bike and not fall off, so that’s a pretty obvious thing you got wrong.

Well your bars spun on the fork so the stem was probably not tight enough. If it stayed in place you probably wouldn’t have gone down. IMO
