Those brakes are a dime a dozen. Finding another pair would be easier that DIYing, which would mean drilling a hole through a bolt. I’m out of town right now, but I’m sure I’ve got some lying around and would be happy to drop one in the mail to you, but the better option would be upgrading those brakes, honestly.

It’s a brake cable adjuster for a caliper brake. Your LBS will sell you a new one, or fish one out of their salvage bin.

You technically don’t even need it if you can get your cable tight enough without it.

Where are you located? I can’t quite read the pantograph on the caliper in your photo but it seems that this is a vintage bike and thusly keeping those brakes would be a good idea if you’re into that type of thing.

It’s called a barrel bolt or barrel adjuster. Those are Altenberger brakes on a bike with a steel brake bridge. That’s pretty low end stuff if that’s the bike you’re trying to repair. You can buy those on ebay.
