
I've experienced many different brake failures, but this was first time caliper ejected by leaving it's body, while brake adapter being intact.
Maybe someone knows why it happened. Was it brake rotor, that failed and ejected the caliper or was it caliper that failed and bent brake rotor?

If I had to guess, you struck something with the rotor that caused it to fold over and in turn it took out the caliper.
My guess would be that one of the brake caliper bolts was loose so the caliper was able to move which caused bending forces on the other mount and shock loading from the movement to the other. Or the rotor bent and busted it all.
Loose caliper bolt -> caliper flexes under load -> rotor bends -> bent rotor increases the force on the caliper -> death spiral until something breaks.
Note the missing valve stem cap, should be an indicator how much maintenance this bike gets.
