    The stack should go race, washer with notch, top hex nut. You need to loosen the hex nut then you can tighten the race by hang. There may be notches for a hook spanner but i cant really tell from the photo. At any rate the stack works together so for tje bottom to move the top mush be loose.

    Edit that notch washer may not be a washer and it maybe the notch for the hook spanner if there is not washer they are a pain to tighten and you need a hook spanner or it will come loose while riding.

  2. You need to unthread the top lockring (hex) and you need a notch spanner that fits in the indentation on threaded race

  3. The best solution here is a new headset. One with wrench flats for both the cup and the lock nut.

    Those tighten by hand cups are next to impossible to get just right. Maybe if you have a hook spanner to keep the intermediate washer from moving as you tighten the lock nut, you can get it right.
