Got a crazy air leak, could basically had to walk, topping up with pump was useless. Upon inspection I see this. Manufacturing defect, or was I just too rough with the stem? New bike, so new tubes. First time using Presta.
Probably the tube got pinched when installing. It gets very tight around the valve. Make sure the valve can move freely (you can press the valve down and pull it out) after installing.

The spokes can poke the tube through the rim if no rim tape

if I had to guess — are you wrenching the presta nut on real tight prior to inflating?
pro tip: the nut does not serve any essential purpose and you don’t need it.

Check the rim behind the rim tape, too. Had one split internally before which nipped a lot like this

Those look like marks from spoke holes. The holes probably have burrs that have cut through the rim tape. Pull rim tape, inspect for burrs, file burrs if necessary, install new rim tape.

Valve wasn’t fully down to the rim. The tube got overstretched filling the void. Happens sometimes with narrow rims.

Might have just rubbed on the spoke bump in the tape. Baby powder can help reduce this friction if that’s the issue.

I really doubt it’s a defect
The evidence of where flat occurred is there now you just have to look and listen without knowing the answer
Tire bead could have protrusion
Wheel rim could have bur
Rim tape could be garbage
Did you use a tire lever during install?
