Chain pins aren’t pressed in very tightly at all. To do that on a chain pin that thing would have to be made of butter. Plus the pin is mushroomed also. If the tool was defective then one or the other part would probably have failed first.
You missed the pin, didn’t realize it and kept cranking. That has to be what happened. Next time back off and realign if the chain doesn’t break easily.
