Had gear skipping with this old road bike in the smallest gear rear gear. Changed the cassette and the chain and the issue still persists. Any tips on what I could do?

You need to adjust the H limiter screw or the barrel adjuster. Have you serviced/adjuster your gears lately?
You just need to tune the RD try searching a tutorial on youtube Good Luck by the way!
Is the hanger straight?
