My wife just mentioned that her pedal felt loose. I thought she meant the retention so I went to tighten it and noticed that it’s the spindle and the pedal body. Any idea why this would happen and what can be done to sort it out?
(Apologies as obviously could use a clean).

Look up the Park Tool video on servicing Shimano pedals. But based on the amount of play, the pedal is potentially dead.
Thats no really play this needs service immediately
