It’s a “Terry” style bike; Virginia Terry was a frame builder who specialized in making frames for women/smaller-statured folks. The smaller front wheel was in the interest of keeping the steering from getting wonky while retaining a larger diameter rear wheel, so it accelerates and holds speed the same as a more conventional 700c bike. I’ve never seen a Miyata in that form, I wonder if it was a special order?

I don’t think that Miyata ever made Terry style bikes, so it looks to me like a 1988 615 with the front wheel replaced.
Super nice frame too, triple butted splined tubing.
