It’s hard to tell, but it appears your non-drive side jam nut may need to be tightened. The clacking sound is it making contact with the inside race of the bearing if that’s the case. The bearings sound okay, but you need to be the judge there. If they feel smooth, they’re good to go.

GREASE!!!!, Anywhere one metal part touches another metal part should be greased, except for your seat post, and stem, every bearing should be packed with grease. That grinding feel/sound will be tiny pits that the balls leave on the race, with grease you have lubrication so those tiny imperfections won’t be as bad. Any time I clean my bike I strip it to individual parts and ultrasonic clean all my bearings and repack them with grease, every thread on every bolt gets greased. I have a bb that is 20 tears old and a headset from when integrals first came out and they run perfectly because of grease.

I would pull the axle, then carefully pry the covers off of the bearings. Clean them out good and put some fresh grease in them. Put it all back together and you’re probably going to be golden.
