This is my Deore axle cone nut, it looks pretty chewed up. How do you think this damage happened? All the balls looks good. Btw this is on my long tail cargo bike that routinely carry about 150lbs in addition to my body weight.
Yeah that’s fubar. I’d be shocked if the cones were ok. Every time I’ve seen this it’s been time for a new hub.

Over tightened or some sand/road reagent got inside. Or bad balls from the start and close to no grease.

That’s decent pitting but I’ve see worse. Definitely due for replacement, but the concern is the hub cups. If this was a standard bike I’d say based on the cone the cups could be okay, as a cargo bike it may be a different story.
ETA: Just saw you asked the cause. Couple things contribute, generally in conjunction with one another. First, over time the grease works its way out as water and debris works its way in. This can create a tiny bit of play, along with general aging the cones and locknuts can back off slightly, permitting more debris ingress and more play in the bearings. Over time they get cross-loaded and driven into the races by each other and ultimately result in the pitting you see here.

Yes, it’s awful. Clean the hub. Replace the bearings. It’s common that the cone is the worst offender. Even if damaged the hub can often be still used. It will depend on a lot of factors and damage. Replace what you can.

I’ve overhauled many hubs where the cones look like this. Usually the hub shell cups are ok. Sometimes they are grooved or pitted too. Clean out the bearings and grease and take a good look at the cup surfaces. If you see damage like those cones have, the hub is due for replacement. Otherwise, get new cones and bearings and ride on. If you are low on cash, just replace the bearings and grease. Adjusted best-as-possible will allow most hubs to operate for another few hundred miles. But it will only continue to wear and eventually grind up the new ball bearings.
