Had my bike setup tubeless at the local trek shop. A few weeks later and the rear tire fails.
Is this a fluke? Or is it somewhat common for this to happen.
Can you elaborate on what you mean by the rear tire failing? Did it blow out on a ride? Or did it come unseated? I’m assuming you were checking pressure and adding air as needed over the course of the few weeks

It seems your tire is not beaded correctly, it can be seen in the second photo.

Tire bead looks like it was nowhere close to being seated in the photos unless you rode it under inflated and it came off the wheel first
