Any water or moisture that manages to get into your frame will creep trough the bottom bracket bearings and dramatically reduce their lifespan. Don’t skip it.
I’ve never seen one like this. Is this for a cup & cone square taper BB?

Is your BB a sealed unit or a cup and cone BB? If it’s sealed you really don’t need one at all, if it’s an old style cup and cone unit you might get some gunk into the bearings by not using one but it will still work just fine.

I’ve made them from PVC pipe before just make sure you make them the right length so it doesn’t rattle or press on the bearings itself

It doesn’t do anything useful. Leave it out. I owned a shop for a while and worked on hundreds of bikes. Some had that and some didn’t. All of them worked fine.

This shops sells those things for $5 if their site is actually accurate
For all the trouble you would go to trying to manufacture your own little sleeve, you could just replace the cup and cone bottom bracket with sealed cartridge bottom bracket like a Shimano UN 300.

You can pick up a Shimano sealed BB for less than $20 and never have to worry about it again
