Is the rear derailleur loose? Can you shake it?
Could it be a spacer from the rear axle? Has the clearance between the small cog and the dropout disappeared?

it looks pretty small like an cable end adapter to one of the eyelets on the frame. If any housing seems loose it could be its missing this can not snug in place

looks like a headset stem cap to me, but not having a ruler next to it makes identification difficult

Rear brake cable have bend over top tube, i would replace it.
How is front brake, very sharp angle there. If such a long stem needed i would run longer front cable housing to prevent sharp angles.

It could be a stop for the front brake housing. Your housings are too short. Watch what happens when you turn the handlebars towards 90 degrees.

Did this bike previously have presta valve tubes? Could be a step down grommet to put a presta tube in a rim drilled for schrader tubes.
Looks like the bushing from your rear brake. Your rear brake looks like it’s missing its bolt.
