I have these old cranks and the only way to tighten them is with the hex in the center.
I would screw it hard, but the more I do the more resistant the cranks are to turn, so I have keep a balance. Is there no way to stop it from becoming loose?

The spindle spacer between the bearings in the bottom bracket is too short or the outside spacers are making contact with the whole bearing. Try a longer spindle spacer first.
