Got the axle out and put it back in so it wasn’t laying around and when I tried to take it back out this happened? I’m new to these type of axles, any help appreciated!

Remove the axle, reinstall the UDH, install the wheel, install the axle only tightening the axle from the non-drive side. Don’t touch the bolt holding on the UDH.
The mirror image pic threw me for a loop, the hanger has come unthreaded from its attachment. You’ll need to reassemble the hanger to the large hollow bolt, and rotate it into place so it hangs below the chainstay(if the bike is upright). Careful inspection of the bolt pattern should show you how it goes together. Are there one or two small bolts or screws missing? Then unthread the thru-axle from the hanger assembly. If you got it together correctly, you should be ready to install your wheel.
