It looks like the bottom bracket bearings are no longer preloaded. Depending on the bottom bracket it either needs to be adjusted or replaced. I would suggest taking it to your LBS.
depends on which kind of bottom bracket it is, but either the bearings are shot, the BB itself is shot, or (best case and probably most likely) the pre-load needs to be retightened.
The BB is dead, you need a new one. No BB that needs side preload has such movement in it. The cranks may move left-right, not up-down.
