Thought I added this as a submission statement… point of the post is- how to get the bolt out and how might it have happened.

Here are my thoughts:
If you have a fine rotary bit, you could try to cut an “x” into the remains of the bolt and use a small Phillips head screwdriver to remove it. I don’t have the fine motor skills to pull that off, but some do!
Barring that, drill it out and use a tap or helicoil to rethread the section. Some knowledge of metric bolt, thread, and tap sizes would be necessary to pull this off successfully.
Why did this happen? Maybe too much torque when tightening paired with time.

You don’t need to drill anything. Pull the other bolt out and then it should fall out the back. Then you can take a vice grip and twist it out
