It is fine, the top cap and top spacer are just here to put some preload before you tighten the stem.

Leave it. The star-nut/spacer assembly serves no purpose other than to load the steerer into the crown and set headset tension. Everything relies on the stem bolts.
When we first saw these in the 90s, lots of dudes would push the star through and remove the top cap after assembly to save precious grams.

100% safe. If the stem can pinch the tube, and the top cap don’t touch the tube, everything will work as intended. This part doesn’t see any additional stress during riding as the stem is holding the steering together.

That looks perfect, you *don’t* want the cut steerer tube level with the top spacer, because you need the gap so when you tension the top cap it’s actually pulling everything together.

If you wanna get granular it’s generally recommended to have 3 mm of space between the top of your spacer and the top of your steerer tube, but yeah looks good to me

My bike came with two spacers underneath the neck, I didn’t like how hight it was but I also don’t have the equipment to cut the neck down so this was my solution to pull everything tight, worked well and now I like the look.

I’d probably put on a top cap.
