    Looks like the main seal/membrane is damaged.
    Shimano did release some spare parts but I’m not sure if they did for road bike parts (there are for mtb)

    That’s the master cylinder head. I believe it could be replaced, but I’d see if Shimano would warranty it to avoid the hassle. These do fail occasionally on Shimano brake levers. It’s possible one of the O rins on it is damaged or not seated properly.

    This is the sort of thing that can happen if you apply pressure or vacuum to the system when bleeding it. I believe there is an o-ring around that plug. If you can get the whole thing out you may be able to reinsert it with the o-ring in place.

    When tightening that part it’s 1nm or less so literally as loose as you can leave it no resistance at all when tightening.
