Last few rides it's gotten pretty bad. Is this a rebuild/replace or can I repack grease and see what happens?

needs new bottom bracket?
Take your chain off. Spin the cranks. If it doesn’t spin freely or sounds like there’s gravel inside get a new bottom bracket. If it spins and there’s no noise check the bearings in your pedals. Spinning them is a good way to do that.
It could be a loose or dray bushing/bearing in one of your pivots otherwise.
