I’m not a frame builder, but it looks like they sketchily stacked two forks on top of each other and welded or brazed a web in between the two. After which they went about attaching the cart.

Its just a standard 700c or 27” fork, with a 20” wheel, and a drum brake.
The shopping cart isn’t attached to the fork at all, its only welded to the frame, so when you turn only the fork will turn.
