When I try to shift into top gear, the mechanism doesn't reach all the way. I've tried regulating the front derailleur and screws that were on the lever, but no luck.
It started to get worse like two months ago after raising the handlebar, but i was ignoring it since it wasn't as that bad (it was harder to switch, but not impossible)
But now i can't do it regardless of force used.
I've ordered a replacement, but i would like to know what went wrong, so it doesn't happen again.
It's Shimano Acera, ST-EF500, front, 3 gears

Often times the small parts within the shifter can get covered with dirt, grime, and grease. I would try blasting it with a lubricant and run through the gears back and forth to work the lubrication into the inner parts. I have had great luck with MPPL (multi purpose penetrative lubricant). Cant guarantee it will solve the problem but not a bad first step. Worst case scenario, you have extended the life of your shifter by lubricating it.
check if the limit screw in the derailleur is making contact, avoiding the derailleur going further up. if the chain is already on the biggest sprocket, turn a couple of turns in the adjust barrel of the cable housing.
The shifter looks to be all junked up, the lever does not spring back into position without you moving it back. Clean the shifter with WD40, do it a couple of times move up and down the gears you will see it will start moving move smoothly. Once running smoothly add some lube. Good luck!!
Rubbing alcohol. Not wd40. Trust me. And pull the cable manually as you go through the clicks.
