I tried to tension them with the spoke tensioning tool but it instantly unscrewed itself… do i just need to change the nipple or do need to switch spokes to. Or is it in the wheel itself?

What to do? Change the spokes? Nipples? Both?

Tension and true the wheel. You will need specialty tools so taking it to a shop may be best or there are some YouTube videos for how to do it yourself.
Keep in mind ‘righty tighty’ here applies to the stationary spoke threading into the nipple. It will seem like it is reverse threaded.
