Going to be doing a bit of a refresh on this old mixte, currently single speed/coaster brake. It would be nice to have a couple gears, but it seems that the 110mm real hub spacing is going to make that a little challenging.

I’m certainly not expecting a ton of range and I’m open to a derailleur or IGH, but I don’t any to spend a ton of money on it either way. Are there any reasonable option here or do I just accept it?

Are you sure you can’t stretch the frame to 120mm? Looks steel. Shimano nexus 3-speed is 120mm. Maybe a freewheel with 5 gears will fit in 120mm with a claw derailleur and a friction (downtube?) shifter too, if you can cut the freewheel spacer on hub axle, but I’m not sure about that last one.

  2. Sturmey archer makes a 2 speed kick shift in 110mm hub spacing, with or without coaster brake. You probably won’t find a pre built wheel like that though

Sturmey Archer and other 3-5 speed variations are available in 110mm. You can increase the range by running a doublw or triple up front, too.
