The axle is bent, this is not uncommon on bikes with freewheel hubs. The load on the axle is distributed unevenly.
You can replace the axle, but to do so requires a thin cone wrench, and a replacement axle for a quick release rear hub with 126mm hub spacing.
Yes, your axle is bent. I’ve seen much worse.
The big trick to using the freewheel remover is to bolt the remover down until you break the freewheel loose.
There is nothing too complicated on the inside. Disassemble carefully because the balls might fall out. Clean the balls and regrease. A cone wrench makes it easier.
Take a picture of the order of the spacers and washers.
