Do you know the make of the stem? It could help id the problem.
My guess is that the keyway that holds the fixed side of the handlebar binder bolt assembly is stripped. You can check by removing both the hex side (bolt) and the smooth side (nut) of the binder bolt from the stem. There will probably be a notch cut into the stem on the nut side that corresponds to a metal protrusion on the nut itself (usually called the key). Ideally, that key should fit in the keyway, and hold the nut in place while you torque down the bolt. What has probably happened is that the keyway got stripped out when someone over tightened the bolt. Those handlebar binder bolts are usually steel, and that stem looks to be aluminum.
If that is the case, I wouldn’t be sure how I would go about fixing that problem. Probably easiest to just get a new stem.
