My crap old bike has the derailleur hanger mounted in the dropout. As a result I have to be very very careful to correctly place the non driveside part of the axle when tightening the axle bolts, otherwise the wheel isn’t aligned and gets stuck between the chainstays. This can be done but is a real pain to get right.
Two questions:
1. Would a dropout spacer help, and do they come in different sizes?
2. Would this have been how the bike was always set up or do you think the previous owner changed anything? I got this bike second hand a while ago and haven’t replaced any parts/tyres etc. It just seems odd that it would have been so tricky to ensure the wheel/axle was in the exact right place every time it needed to be taken off.

Nah, that’s standard practice for those style of dropouts using a claw mount derailleur.
The drive-side axle is fixed in place as far back as it goes against the claw mount.
Then you align the wheel relative to the chainstays/seat stays. Then tighten the non-drive-side nut.
They do make spacers if you find this to be a pain though.
