  1. Compare what you have vs. the product picture here: https://www.somafabshop.com/shop/948015-soma-thru-axle-12×100-for-fog-cutter-smoothie-hp-carbon-forks-6853?search=smoothie#attr=

We can’t tell, because your picture is blurry, whether you do have the sleeve on the TA. Hard to tell, but it looks like the nut for the TA fits a hole with a larger diameter?* What happens if you flip the TA around so the side we are looking at in your pic is on the non disc side?

Edit: According to the diagram on the page above, the nut flange fits in a bore of ~19mm, the sleeve that sits under the bolt head fits a bore of ~15mm. Try reversing the TA.

  2. do refer to the indentations made by the thruaxle?
These are normal, but normally there would ne no paintclose ro the axle hole. this is to ensure a solid surface for the axle to fasten against. paint can always chip and than the fastend axle is not fastend anymore.
there are special tools for facing these mounting points.

  3. It’s the wrong diameter. Does the manufacturer state what size it should be? as some earlier thru axle framed road bikes were 15mm not 12mm.
