Is the tire centered in the fork? Is the front brake operating well? If yes to these, you have a bent rack: no biggie- bend it straight.
Could be: Axle not fully seated (check the spring direction), rack mounted crooked, fork is bent.
IMO it is unlikely to be an out-of-true wheel. It would have to be way off in that particular position to look like this, and that would require it to be tacoed badly enough it would not look as straight as it does in that pic.
To my eyes the wheel is aligned and the rack is crooked. If you’re using the rack as a sighting tool are you sure the rack is straight?

An untrue wheel would not get this result.
The hub could be seated wrong in the fork, but that seems unlikely, but it is the easiest thing to check.
I think it is the rack that is bent, you should be able to bend it back.
That’s the front wheel. The steering of the forks and handlebars rotates in relation to the frame.

Rack looks crooked over wheel… I’ve found that the few racks I’ve installed, either were types that every connector/movable point needed fine “tweeking,” or some just weren’t manufactured 100% true/straight, or bent in shipping, etc, and had to bend back if/what I could.
I have this same rack (from Amazon). Took some wiggling to get it centered.
