Wierd chain derailing

My coworker (works in a bike shop) and I have both been trouble shooting this. I've messed with all the derailer fine tuning screws. We've replaced the chain. (Old chain had a bum link) but this keeps happening. Any thoughts are much appreciated.

Edit: it occurred to me that the small cog might just be finally worn down enough that it's throwing the chain.
