On my ride today I noticed when the chain was on the 5th gear (from the top) it made a slight "grinding" sound. I'm definitely going to clean it but I stopped by my local shop and he tried to explain the adjustment to me but he did it on a bike that the rear looked different. So, I am not sure what to try?
Also, the teeth looked OK to me.
Any help is appreciated.

Try turning the barrel adjuster some small turns anti clockwise. That’s easier than checking your hanger or changing the cable which also could be the issue.
It looks quite clean. It might be shift cable tension. When there is too much or too little pull on the derailleur the chain van be slightly off the cog.
There is a barrel adjuster on your shift lever, where the cable housing goes into the shifter.
If you turn that out of the shifter, the cable housing becomes a bit longer so the shifter cable gets more tension. Screw the barrel adjuster in and you decrease cable tension.
Do that while the rear wheel is off the ground and pedaling gently forward. Adjust only in small increments and listen if the grinding disappears or becomes louder. If that helps, shift up and down the cassette to see if you didn’t change the overall shifting.
