You didn’t perform a basic pre-ride inspection to be sure that everything was tight. The crank bolt(s) came undone. Catastrophic sh*t can happen if you don’t check your bike before you ride.

A good tip is Preventative Maintenance, Check if all your boys and screws are tight before a ride if not regularly , I caught my pedal behind loose from vibrations before something like this happened.
Same happened to me on my new bike. I heard a cracking noise but I was shure it came from the saddle.
That day I learned to not ignore these things on my bike.

I’ve done this before. Once it’s loose, it only takes a few rotations to come off.
This happened after I recently swapped the left crank arm with an equivalent because the pedals stripped their threads. My error was I didn’t press the right crank arm towards center which didn’t give me quite enough to pinch too with the left crank arm pinch bolts and the cap screw.
Correct order would be to loosen both pinch bolts, press right crank arm in, then screw in the left side cap screw, then tighten your pinch bolts to spec. Spec is usually engraved on the crank arm somewhere.

happened to me before on my tri bike.
i did some self service to my bike(crank included)prior that i might not tighten it properly.
i did hear something before it happened that i assume just my saddle creak 😔
since then i tripple check 🫣
